Breast Cancer – As background, the EAZ171 study’s primary results showed that Black patients with early-stage breast cancer who received docetaxel chemotherapy every 3 weeks were less likely to develop peripheral neuropathy or require dose reductions than those who received weekly paclitaxel chemotherapy. A new analysis focuses on patient-reported outcomes (PRO) data. Breast Cancer – This analysis pooled data from two legacy cooperative group trials in patients with clinically defined good-risk ductal carcinoma in situ (DCIS). Good risk was defined as low- or intermediate-grade lesions ≤2.5 cm with negative margins ≥3 mm. This analysis examined the role of tamoxifen in patients who underwent lumpectomy alone (without radiation therapy). Tamoxifen use was associated with a significant reduction in the risk of recurrence in the same breast over nearly 15 years of follow-up, driven primarily by fewer invasive recurrences. Cancer Care Delivery Research – For cancer patients, continuing to smoke during treatment may shorten survival. Despite longstanding clinical practice guidelines recommending tobacco treatment as part of cancer care, most patients are only briefly advised to quit (“you need to quit”) and are rarely given support to do so. A nationwide trial found that 28.4% of patients who received 6 months of telehealth counseling and nicotine replacement therapy during cancer treatment were tobacco-free after 6 months, compared with 14.7% of patients referred to a Quitline without further support. This approach, implemented in community oncology settings, resulted in higher participation (81% versus 5%), increased medication use (83% versus 49.5%), and greater patient satisfaction (90% versus 40%). The results show that integration of effective tobacco treatment into cancer care is both feasible and highly effective. Cancer Control and Survivorship – VEGFR (vascular endothelial growth factor receptor) tyrosine kinase inhibitors block signals that tumors use to grow new blood vessels (angiogenesis) and are used in cancer treatment. However, they often cause hypertension, a dose-limiting side effect. CARISMA (EAQ191) is the first randomized trial to investigate this issue, finding that intensive control of systolic blood pressure (target <120 mm Hg) is feasible, safe, and well tolerated. The approach lowered blood pressure without increasing adverse events or worsening patient-reported outcomes compared to usual care. Leukemia – Up to 70% of new acute myeloid leukemia (AML) cases occur in older patients. The E2906 randomized phase 3 trial focused on maintenance treatment for patients above the age of 60 who were in remission. It's results serve as a key data set: primary results were first reported in 2015, followed by an update in 2019, and quality-of-life data in 2023. Lung Cancer – The phase 3 randomized EA5142 (ALCHEMIST) trial evaluated adjuvant nivolumab after surgery and planned chemotherapy and/or radiotherapy in patients with resected non–small cell lung cancer (NSCLC) without sensitizing EGFR or ALK alterations. Primary results show that adjuvant nivolumab was not associated with improved disease-free survival overall or in the subgroup of patients with high PD-L1 expression (≥50%). Melanoma – This translational study, which utilized resources from the legacy E1609 trial, demonstrated that combining immune biomarkers from blood and tumor tissue generated a baseline signature that more accurately predicted which patients with high-risk melanoma would benefit from adjuvant immunotherapy than either biomarker source alone. Myeloma – The newly published ENDURANCE (E1A11) trial findings provide the first randomized evidence on the optimal duration of lenalidomide maintenance for patients with newly diagnosed multiple myeloma. The study found more toxicity and no survival benefit from lenalidomide maintenance given until disease progression compared to stopping after 2 years in patients who did not undergo a stem cell transplant. NCI-MATCH – Arm Z1I investigated adavosertib, a selective, ATP-competitive, and small-molecule WEE1 kinase inhibitor, in patients with heavily pretreated advanced solid tumors with variants in BRCA1/2. The study did not meet the primary end point.
